of death, and some proof of death, such as a newspaper obituary, death
certificate, photocopy from a biographical reference volume or
encyclopedia or magazine. Mention the Freedom of Information Act.
Agree to pay reasonable costs up to $20 if necessary.
It takes a while, but is worth the wait.
